TERMS AND CONDITIONS AND COMPLAINT PROCEDURE
Terms and Conditions for the implementation of closed educational events valid from 30 September 2025, organized by MDDr. Hana Žižková (hereinafter referred to as the Provider).
These Terms and Conditions regulate the mutual relations between the purchasers of the theoretical educational event PedodonticsPro (hereinafter referred to as the Purchaser) and their Provider.
I. Definitions
- The Provider’s educational events include lifelong education of dentists in the form of theoretical and practical training courses. The Provider is MDDr. Hana Žižková, Zahradní 380/10, Samotišky, 779 00. Company ID 19549270, business licence issued in Olomouc on 24 July 2023 under file no. SMOL/216814/2023/OZIVN/RGR/Bla.
II. Order
- The Purchaser shall complete the Order in electronic form, including all data and particulars (in accordance with the Commercial Register if the entity is registered in it). The Order submitted by the Purchaser is binding.
- The Order is confirmed by the Provider of the educational event without delay.
- By sending or submitting the Order, the Purchaser confirms the accuracy and truthfulness of the data and also confirms their consent to and acknowledgment of the Provider’s Terms and Conditions.
- If there is any change to the data relating to the Order on the part of the Purchaser, the Provider must be notified of such fact without delay.
- By registering for the educational event, the Purchaser gives consent to the Provider to process the personal data of participants in accordance with the relevant provisions of Act No. 101/2000 Coll., as amended, to collect, process and store the personal data stated in the Order for an indefinite period of time. Personal data include, in particular, the identification data of the participants (name, surname, academic title), identification data of the organisation (organisation name, Company ID, VAT number, address, etc.). Personal data will be processed by the Provider to the extent necessary for the organisation of the course and for evidence of educational events and attendance. The Purchaser is entitled to withdraw such consent at any time by delivering a written withdrawal of consent to the Provider’s registered office.
III. Payment terms
When ordering an educational event, the Provider shall issue an invoice in the amount of 100% of the course price. The Purchaser is obliged to pay according to the instructions stated on the invoice.
IV. Withdrawal from the Order
Cancellation conditions
If a registered person is unable to attend the course, they may be substituted by another person free of charge. If the Purchaser requests a refund, the conditions are as follows:
- Theoretical part – two weeks or more before the event – cancellation fee 0%
- Theoretical part – less than 2 weeks before the event – cancellation fee 100%
V. Other provisions
- The Provider reserves the right to adjust the programme of the educational event and the lecture premises. The Purchaser will be informed of this in due time.
VI. Scope and conditions of complaints
- The customer has the right to file a complaint to the Provider regarding the educational event due to:
- event organisation
- methodical content of the event
- The Purchaser shall submit the complaint in writing immediately after discovering the reason for the complaint.
- The complaint must contain:
- contact details (Company ID, company name, registered office address)
- name and surname of the event participant or the person submitting the complaint on behalf of the customer
- identification of the event – date, title, and, if applicable, invoice number related to the event
- description – reason for complaint
If the customer’s written complaint does not contain the above-mentioned information necessary for a proper complaint submission, it will not be accepted for further processing.
- Method and deadline for handling complaints: The Provider is obliged to handle the complaint no later than 30 days from the date of receipt of the complaint from the Purchaser. The date of receipt is understood as the date the complaint was delivered to the Provider. If the Provider acknowledges the complaint as justified, it shall promptly rectify the situation and offer the Purchaser adequate compensation, whether in the form of replacement performance or financial compensation, based on agreement and with regard to the Purchaser’s preference.
